FDSA Lapu-Lapu City Campus is now accepting ONLINE ENROLLMENT for NEW/FORMER STUDENTS in the FIRST SEMESTER of FISCAL YEAR 2020-2021.

ONLINE APPLICATION PROCESS:

1. Sign in to Gmail account and accomplish FDSA Enrollment Form using the link provided in the description below.
2. Read the instructions carefully and fill-in the information needed.
3. More preferably for verification purposes, attach clear photo(s) of Report Card/Transcript of Records showing your name and computed General Weighted Average (GWA) in “Add File” button under “Credentials”. - (Applicable for New Students or Transferees)
4. If the credential is not yet available, you may still apply.
